The purpose of the present study was to determine the difference in the effectiveness of Islamic spiritual therapy and narrative therapy based on Surah Yusuf on the moral foundations of students. In terms of purpose, this study was applied, and methodologically, it employed a quasi-experimental, single-factor design with two levels and three groups, evaluated through a mixed-method approach. The research sample consisted of 60 undergraduate students from Tehran Medical Sciences, Islamic Azad University, in the academic year 2024–2025 , selected via purposive sampling and assigned to three groups. The experimental groups completed the Moral Foundations Questionnaire (MFQ) by Haidt and Graham (2007) across three assessment phases. Statistical tests were utilized for data analysis, and the data were processed using SPSS software (version 26). The findings revealed that both interventions—Islamic spiritual therapy and narrative therapy based on the Quranic stories of Surah Yusuf—were effective in improving the moral foundations of the students. However, since the primary objective of this study was to compare the efficacy of these two therapeutic approaches, the comparative analyses and post-hoc tests demonstrated a significant difference between the impact of the two interventions; specifically, Islamic spiritual therapy was significantly more effective in promoting students’ moral foundations compared to narrative therapy based on Surah Yusuf (p<0.05). Furthermore, the follow-up phase confirmed that this superiority and positive effect remained stable over time
